College Application Essay?

I am applying to Bridgewater State, Keene State, and Plymouth State. how do i format my essays? do i need a cover page, if not then where do i put my title? is there a specific way i should do it? normally i would put a title centered on the first page and a header with my name and page number on the topright corner on each page, any comments would be greatly appreciated.
